Clinical Liaison (LPN)

Location: Gulfport, MS

St. Joseph Hospice located in Gulfport, MS is seeking a Full-Time Clinical Liaison (LPN) to join our team.

Requirements:

  • Licensed Practical Nurse (LPN) in the State of Louisiana with a minimum of 2 years’ experience
  • Have a professional demeanor with excellent communications skills
  • Be flexible, self-driven, accountable for results, autonomous, yet supportive of team efforts.
  • Have a valid driver’s license, proof of valid auto insurance, and active clinical license are required.
  • Prior sales or business development experience, preferred
  • Hospice or Home Health experience preferred
  • HCS Interactant software experience preferred, but not required

Qualifications:

  • Conducts onsite patient visits to review physician orders, assess clinical needs, and collect referral information
  • Determines patient eligibility for Hospice, Home Health, or AIM Palliative services
  • Facilitates smooth transitions to home through patient education and care coordination
  • Collaborates with leadership and clinical teams to identify high-risk patients and implement plans of care
  • Focused on reducing 30-day hospital readmissions and preventing exacerbations
  • Manages facility-based patients, hospital-hold patients, and new admissions
  • Ensures physician oversight and obtains required orders for hospice plan of care
  • Completes accurate face-to-face electronic documentation and communicates with appropriate agency staff
  • Establishes, grows, and maintains strong relationships with referral sources
  • Participates in community outreach initiatives, including health education events
  • Maintains strict adherence to HIPAA, compliance guidelines, company policies, and regulatory standards
  • Promotes organizational core values and promptly reports compliance concerns
